Zusammenfassung
Das Alexianer-Krankenhaus Krefeld hat ein Zentrum zur Behandlung der Posttraumatischen Belastungsstörung (PTBS) eröffnet. Schrittweise konnte das Behandlungsangebot erweitert werden. Jetzt liegt ein geschlossenes Konzept vor, dass die Behandlung, die Prävention, die Lehre und die Forschung institutionell auf einen Nenner bringen. Die Behandlung der PTBS beruht auf der Mehrdimensionalen Psychodynamischen Traumatherapie (MPTT). Hierbei handelt es sich um ein manualisiertes, tiefenpsychologisch fundiertes Verfahren, das schulenübergreifend komplementäre Therapieformen integriert. Das Organisationskonzept des Zentrums ist vierschichtig. Es besteht aus einer stationären Behandlungseinheit, der Komplex-Liaison, einer Ambulanz für Gewalt- und Unfallopfer sowie der Integrativen Psychiatrischen Behandlung (IPB). Die IPB ist eine Krankenhausbehandlung ohne Krankenhausbett. Ein mobiles multiprofessionelles Team führt eine spezifische psychotraumatologische Behandlung zum Teil im häuslichen Umfeld des Patienten durch. In der Komplex-Liaison kann diese Form der Behandlung am Krankenbett einer somatischen Fachdisziplin erfolgen.

Das Behandlungskonzept wird von der so genannten Zielgruppenorientierten Intervention zur Früherkennung und Prävention der PTBS flankiert. Hierbei handelt es sich um ein Kriseninterventionskonzept, das auf einer Erhebung von Risikofaktoren basiert, um ein individuelles abgestuftes Interventionsprogramm nach schwerwiegenden Lebensereignissen einzuleiten. Lehre und Forschung haben am Zentrum für Psychotraumatologie einen besonderen Stellenwert. Seit Sommersemester 2004 fungiert das Zentrum als Lehr- und Forschungsabteilung der Universität zu Köln – Institut für Klinische Psychologie und Psychotherapie. Das Lehrangebot konnte für angehende Psychologen um praxisorientierte Seminare erweitert werden. Im Mittelpunkt der Forschung steht die Evaluation der Versorgungskette von Betroffenen, die als Folge von sexualisierter Gewalt, häuslicher Gewalt, Arbeitsunfällen, Natur- und Kriegskatastrophen und ähnlichen schwerwiegenden Lebensereignissen unter einer Belastungsstörung leiden. An diesem Organisations- und Versorgungskonzept wird deutlich, dass es für eine ätiologiespezifische Behandlung der PTBS erforderlich ist, etablierte Strukturen im Gesundheitssystem zu modifizieren. Dies bezieht sich sowohl auf die Polarisierung von Therapieverfahren unterschiedlicher Schulen als auch auf die klassische Trennung von ambulanter und stationärer Versorgung. Darüber hinaus liefert die Teilhabe an Lehre- und Forschung ideale Voraussetzung die Früherkennung, Behandlung und Fallsteuerung dieser Patientengruppe auf einer wissenschaftlichen Grundlage zu optimieren.

Centre of Psychotraumatology at Alexianer-Hospitel Krefeld: therapeutic strategies and health care management

Summary
The Alexianer-Hospitel Krefeld has inaugurated a centre for treatment of posttraumatic stress disorder (PTSD). The medical attendance has following outline. First, the therapy is based on the manualized multidemsional psychodynamic treatment of traumatic stress (MPTT) by Fischer. MPTT integrates elements from diverse psychotherapeutic schools. The department consists of four parts: first, a ward for inpatients and a ward for outpatients, further, integrative psychiatric and complex-liaison treatment. This is possible by a mobile multiprofessional team. The integrative psychotraumatological approach is based on the home-treatment idea. The complex-liaison can be regarded as a modification of integrated therapy. Multiprofessional psychotraumatic treatment starts right after the traumatic event while the patient is still in the sick-bed of departments from other disciplines such as surgery or internal medicine.
The therapy is supported by a Target-Group-Intervention-Program (TGIP). The essential component of TGIP is an intervention schedule designed to prevent PTSD through early identification of risk factors. According to the risk profile varying intervention measures such as psychological first aid, psychoeducation, diagnosis of PTSD and psychotherapy are recommended and standardized. Research and training have a special significants. The Centre of Psychotraumatology cooperates with the University of Cologne – Institute of Clinical Psychology and Psychotherapy, so that practical training in Krefeld is part of the education of psychology students. Main interest of research is the benchmarking of PTSD victims. This includes the evaluation of psychodynamic orientated treatment, effectivity of crisis interventions and rehabilitation. We conclude that according to the aetiology of PTSD a successful attendance requires the modification of analytic psychotherapeutic elements. Further, the strict division of in- and outpatients treatment should be raised. Over this the combination of clinical approach with research and teaching meet the requirements of modern concepts of psychotraumatological health care.

Zusammenfassung
Es werden Ergebnisse einer qualitativen, explorativen Untersuchung zur Frage referiert, welche situativen oder personenbezogenen Faktoren eine eher stützende Beratung bzw. Trauma-Akuttherapie ermöglichen und welche andererseits dazu führen, dass auch akut traumatisierte Patienten einer längerfristigen Behandlung bedürfen. Die Studie bewegt sich im Rahmen der „Mehrdimensionalen Psychodynamischen Traumatherapie“ (MPTT) und der differentiellen Indikation nach diesem Verfahren. Sie führt u.a. zu dem Ergebnis, dass bei Vorliegen lebensgeschichtlicher Vortraumatisierungen auch bei akuttraumatisierten Personen eine der weiterführenden MPTT-Varianten („mittelfristiger“ bzw. „langfristiger traumatischer Prozess“) indiziert ist.

Chances of and limits to self-healing and short-term therapy in the case of acutely traumatized patients

Summary
This article provides results of qualitative explorative research upon the question which situational and personal factors of patients enable a mere supportive counsel with respect to acute trauma therapy. On the other hand, it also investigates the factors that lead to having acutely traumatized patients undergo long-term treatment. The study draws upon the issue of “Multi-dimensional Psychodynamic Trauma Therapy” (MPTT) and the differentiated indications related to this method. The study concludes that either of the MPTT approaches (mid-term or long-term traumatic processes, respectively) is also applied in cases of such acutely traumatized individuals that do have a personal history of previous traumatization.

Eine bibliotherapeutische Selbsthilfeschrift wird am Therapieverlauf einer Patientin eingeführt, die zu Therapiebeginn die Kriterien des Psychotraumatischen Belastungssyndroms (PTBS) erfüllt. Die wichtigsten Ergebnisse und Veränderungsschritte der Therapie werden aufgezeigt und diskutiert. Die bibliotherapeutische Arbeit mit einer geeigneten Anleitung zur Selbsthilfe scheint den traumatisch bedingten Kontrollverlust zu überwinden und dem erhöhten Bedürfnis nach Eigenkontrolle bei traumatisierten Patienten entgegen zu kommen. Bei dieser Patientin verbindet sich die akute Anpassungsproblematik mit einer adoleszenten Ablösungskrise von ihrer Ursprungsfamilie. Das Angebot von Eigenaktivität, das die Broschüre eröffnet, wirkt sich hier besonders günstig aus. Kriterien zur differentiellen Indikation und Einführung des Selbsthilfemanuals bei Traumapatienten werden entwickelt. Ergebnisse einer explorativen Studie zu Wirkung einer Trauma-Selbsthilfebroschüre in drei unterschiedlichen Anwendungsformen (Eigenanwendung, Beratung, Psychotherapie) werden vorgestellt. Es werden erste Empfehlungen für den Einsatz der Broschüre in psychotraumatologisch fundierter Beratung und Psychotherapie formuliert .Vom Einsatz wissenschaftlich nicht geprüfter „Ratgeber“ bei traumatisierten Personen raten die Autoren ab.

The application of bibliographic material aiming at self-help in work with acute traumatized patients

Summary
This article introduces bibliographic material aiming at self-help in work with acute traumatized patients. This is being demonstrated by the therapy of a female patient, extending over 31 one-hour sessions. At the start of her therapy, she showed the essential criteria of Psychotraumatic Stress Disorder (PTSD). A self-help manual for acute traumatic reactions was integrated into the therapy. The most important results and the stages of the change process are discussed here from a psychodynamic point of view. Apparently, „active bibliotherapy” by means of a self-help guideline proved effective. This aided the patient to overcome the feelings of loss of control that were induced by the trauma. It also helped her to meet her need for self-control, which had been exacerbated by her recent struggle for separation from her parental family. Differential indications for integrating self-help material in trauma therapy are being discussed. On the basis of an exploratory empirical study recommandations are beeing formulated for introduction and use of bibliographic material in counseling and therapy with traumatized patients. According to the authors critical reservations are indicated regarding the use of bibliographic material not tested in empirical research.

Zusammenfassung
In Sartres autobiographischer Schrift „die Wörter“ (les mots) werden Spuren einer frühkindlichen Traumatisierung mit der Erfahrung von Todesnähe untersucht. Ansätze kreativer Verarbeitung des frühen Trennungstraumas werden am philosophischen und literarischen Werk des Autors aufgezeigt. Zwar war es Sartre nicht möglich, die frühe Traumatisierung in seiner autobiographischen Schrift zu erreichen, da sie zu einem Zeitpunkt stattfand, zu dem das „verbale Selbst“ sich erst zu bilden beginnt. In diese präverbale Erfahrung reichen „die Wörter! „ nicht hinein. Dennoch verdanken wir Sartre wichtige philosophische Beiträge zur phänomenologischen und literarischen Beschreibung traumatischer Erfahrung und philosophischen Begriffsbildung in der Psychotraumatologie. Es wird zwischen dem phänomenologischen Ansatz des Philosophen Sartre und seiner Ontologie unterschieden. Während sich die phänomenologischen Beiträge als eine subtile Beschreibung und Reflexion traumatischer Erfahrung erweisen, wird seine Ontologie als unbewusste Wiedergabe und Reproduktion des Traumas und seiner Folgen für das Selbsterleben verstanden.

Geworfenheit und Entwurf – Trauma and Creativity in Jean-Paul Sartre

Summary
In Sartre’s autobiographical essay Words, traces of early childhood traumatization in conjunction with the close experience of death are examined. The beginnings of a creative coming-to-terms with the trauma of early separation are demonstrated in the author’s philosophical and literary works. It was, of course, not possible for Sartre to arrive at the early traumatization in his autobiographical essay since it occurred at a point in time at which the formation of the “verbal self” had only just begun. His was a pre-verbal experience into which Words could not reach. Still we are indebted to Sartre for important philosophical contributions to the phenomenological and literary description of traumatic experience and for philosophical concepts in psychotraumatology. A distinction is made between the phenomenological approach of Sartre the philosopher and his ontology. While the phenomenological contributions prove to be a subtle description of and reflection on traumatic experience, his ontology is to be understood as the unconscious reproduction of the trauma and its consequences for the experience of self.

Zusammenfassung
Selbsthilfeaktivitäten stellen für psychisch traumatisierte Menschen eine wichtige Ressource in der Gesamtheit therapeutischer Angebote dar. Allerdings können aufgrund der besonderen Psychodynamik dieser Patientengruppe traditionelle Methoden und Konzepte der Selbsthilfe nicht unreflektiert adaptiert werden, sondern bedürfen einer ätiologiespezifischen Modifikation. In dem vorliegenden Beitrag wird zunächst die Notwendigkeit einer gegenwarts- und zukunftsorientierten Arbeit in der geleiteten Selbsthilfe begründet und daraus ein Modell für Selbsthilfegruppen entwickelt, das sich auf Erfahrungen in Gruppen der Klinik für psychotherapeutische und psychosomatische Medizin im Ev. Johannes-Krankenhaus Bielefeld stützt. Anschließend werden Vorschläge für die therapeutische Praxis aufgezeigt, die konkrete Hinweise enthalten, wie in der Traumatherapie Selbsthilfeelemente eingebracht bzw. verwendet werden können.

Psychological traumatization and self-help: A report on practical experience

Summary
Among therapeutic offers as a whole, self-help activities represent an important resource for traumatized persons. Due to the special psychodynamics of this patient group, however, traditional methods and concepts of self-help cannot be adapted unreservedly but rather must undergo case-specific etiological modifications. In the following article the necessity for immediate and future oriented efforts in guided self-help is demonstrated and a model for self-help groups developed which is based on experience with such groups in the Clinic for Psychotherapeutic and Psychosomatic Medicine in the Ev. Johannes Hospital in Bielefeld. In conclusion, suggestions for the psychotherapeutic practice are given containing precise instructions on the integration and use of self-help elements in trauma therapy.

Zusammenfassung
Ein Kinderbuch, das kleine Kinder mit den Lebensgewohnheiten von Vögeln vertraut macht, wird einmal als Anleitung zu „frühem Lernen“ eingeführt, zum anderen als Bestandteil einer Anleitung zur Selbsthilfe, schließlich auch als mögliches „Medium“ einer Traumatherapie bei Kindern im Kleinkind- und Vorschulalter. Der Beitrag ist so verfasst, dass Eltern und Erzieher ihn unmittelbar als Anleitung zu Selbsthilfemaßnahmen bei belasteten und traumagefährdeten Kindern verwenden können. Am Beispiel heute fast endemisch verbreiteter Verhaltensstörungen vom Phänotyp der sog. „Aufmerksamkeitsdefizit-/Hyperaktivitätsstörung“ (ADHS) wird ein polyätiologischer Zugang ausgeführt, der günstige Voraussetzungen sowohl für Selbsthilfe als auch für traumatherapeutische Maßnahmen bietet.

Playful Learning in Childhood – Instructions for early learning and self-help after situations of stress

Summary
A children’s book that introduces children to the world of birds and gives instructions for early learning. At the same time it shows ways of self-help and is a possible medium in the therapy of traumatised toddlers and nursery school children. This publication is written, so that parents and teachers can use it as a self-help guide for stressed and trauma endangered children. Using today’s widespread phenomenon of children with behavioural problems, attention deficit syndrome and hyperactivity as an example, a variety of different ways are shown to create an environment for self-help and therapy.
